Insurance:
All major medical plans accepted including Medicare.
Payment Options:
Our policy is that all co-pays and deductibles are due at the time of check in, as well as private pay. All Medicare only patients will be responsible for their 20% at the time of service. Unpaid deductibles and co-insurance percentages will also be due upon checking in. As a courtesy to our patients, we will file claims for all insurance types. We do not accept third party insurances, letter of protections or automobile claims. We do accept cash, checks, and credit cards. In the event that your bank returns payment made by a personal check, a service fee of $40.00 will be billed to your account.
Appointments:
Please bring MRI reports/films and relevant medical records to the appointment.
Scheduling
To schedule an new patient or follow up appointment, contact our office.
Cancellation
If you are unable to keep your scheduled appointment, please provide at least 24-hours notice by contacting us.
Late
If you are more than 15 minutes late for your appointment, you may be rescheduled for a later date.
Emergencies:
A physician is available 24-hours/7-days a week. Please contact our office if you need to speak with a physician after hours. If you have a serious medical emergency, please go to the nearest emergency room and have our on-call physician contacted.
Additional Information:
It is important that you bring along with you all previous pertinent medical records including CT scans and MRI reports to ensure a complete evaluation.
We also ask that you bring with you all medications that you are currently taking in their original containers.
